The code is a SQL Forum system. It includes a SQL dump to provide the SQL structure (since it uses multiple tables for things) as well as the code. COmmands given are listed in a document in the zip file. Also included is a vital functions file and an administrative tool that can be either dished out or shoved in the master room and @locked to those you want to be able to use it.

Patchlevel/version of server required/tested on: 1.8.0ish
Privileges required: Dump gives all the privleges required. But all three objects should be given wizbits.

Support policy: Full support, Email, En-M*U*S*H support and if the issue is big enough I will connect to your site and fix it with extreme prejuduce 

Installation instructions: quote the dump, set &funs on the admin object and the command object to the dbref of the function object. Put the forum command object in your master room and throw the function object in cold storage. Make sure the command object is !no_command. Do with the the admin object as you wish. Either in the master object with a @lock/use or use it as a parent for little admin tools.
